The consumer stated that while cooking chicken in the pan with the burner on low/med, she put the glass pan cover on the pan and not even a minute later, it shattered all over.

I was using a non-stick cooker with a tempered glass lid to make a chicken stew on a gas stovetop. The stew had been cooking for several hours. The flame was turned to low. Suddenly there was a load bang & the glass lid had exploded/shattered. Glass shards were thrown.
